Semen analysis explained: what the results can tell you
A clear introduction to semen analysis, common measures and questions to take to a fertility appointment.
Male factors can contribute to difficulties conceiving, so evaluating both partners where relevant can avoid an incomplete picture. Semen analysis is a common starting investigation, but it is not a pass-or-fail fertility test.
What is measured?
A laboratory may report semen volume, sperm concentration, total sperm number, motility and morphology. Results are interpreted against laboratory methods and clinical context. A value outside a reference limit does not necessarily mean pregnancy is impossible.
Why repeat testing?
Semen measures can vary between samples. Illness, fever, collection conditions, abstinence interval and other factors may affect results. Clinicians may recommend repeating a test before drawing conclusions.
What else might matter?
Relevant history can include medications, testosterone or anabolic steroid use, previous infections, surgery, heat exposure and symptoms that merit examination. Some situations warrant specialist andrology or urology advice.
Useful questions to ask
Ask which measures were outside reference limits, whether repeat testing is indicated, whether any examination or hormonal assessment is appropriate and how the result fits alongside the wider fertility history.
Fertility Barrier Finder can help organise possible factors and questions, but does not diagnose a male fertility condition.
