Normal Ultrasound but Suspected Endometriosis: What Next?
Short answer: a normal ultrasound does not rule out endometriosis. The most useful next step depends on symptoms, whether you're trying to conceive and what clinical question needs answering.
Why can an ultrasound be normal?
Ultrasound can identify ovarian endometriomas and some deep endometriosis, especially when performed by a specialist. Smaller superficial deposits can be difficult to detect. NICE specifically advises against excluding endometriosis when examination and ultrasound are normal.
What symptoms should you discuss?
Tell your clinician about pelvic pain, painful periods, pain during or after sex, cyclical bowel or bladder problems, and how symptoms affect everyday life. Symptoms are important even when imaging looks reassuring, but they are not diagnostic by themselves.
What investigations come next?
Depending on symptoms and local pathways, clinicians may consider specialist transvaginal ultrasound, MRI for suspected deep disease or a referral to gynaecology. In selected cases laparoscopy may be discussed. The right step depends on clinical circumstances, previous imaging and how results could change management. A normal scan is not a reason to dismiss persistent symptoms.
How does fertility planning change the conversation?
If conception is a goal, ask whether a fertility investigation should assess other factors in parallel, including ovulation, tubal patency where indicated, and a semen analysis when relevant. Surgery is not automatically the best fertility intervention, and decisions should balance symptoms, fertility priorities and potential effects on ovarian reserve.
Five questions for your appointment
- What type of ultrasound did I have, and was it performed by someone experienced in endometriosis imaging?
- Could my symptoms justify a specialist referral even with a normal scan?
- Would more imaging change management?
- Should we discuss tubal testing or other fertility factors?
- What are the benefits and trade-offs of each option for my fertility goals?
